JACKIE GOMEZ WITH JACK AND FRANK SALON ARE SO EXCITED YOU MADE THE DECISION TO PROCEED WITH HAIR EXTENSIONS! PLEASE READ THE FOLLOWING PRIOR TO YOUR APPOINTMENT AS THIS IS BINDING. BY PROCEEDING WITH YOUR EXTENSION APPLICATION YOU AGREE TO ABIDE BY JACKIE GOMEZ'S IN ACCORDANCE WITH JACK AND FRANK SALON'S HAIR EXTENSION POLICY.

AS A CLIENT OF JACKIE GOMEZ, I UNDERSTAND THAT I WILL BE PAYING A NON-REFUNDABLE DEPOSIT TO COVER COST OF HAIR AND THE AMOUNT OF TIME BLOCKED FOR MY APPOINTMENT. THE REMAINING BALANCE WILL BE DUE THE DAY OF SERVICE.

I ACKNOWLEDGE THAT HAIR EXTENSIONS ARE SENSITIVE AND DIFFERENT FROM INTACT HUMAN HAIR AND HAVE BEEN MADE AWARE OF PROPER CARE AND MAINTENANCE.

IN THE EVENT THAT YOU, THE CLIENT,  DOES NOT DECIDE TO NOT KEEP THE EXTENSIONS APPLIED AFTER YOUR CONSULTATION, YOU, THE CLIENT, ARE FULLY RESPONSIBLE FOR THE TOTAL PAYMENT RENDERED.

AS A CLIENT OF JACKIE GOMEZ, I AM  AWARE THAT PURCHASED HAIR MUST BE PAID FOR PRIOR TO SERVICE AND IS NON-REFUNDABLE.

I AM  AWARE WITH PROPER CARE OF MY EXTENSIONS, THEY SHOULD LAST UNTIL NEXT MAINTENANCE MOVE-UP. (6-8 WEEKS) AND UNDERSTAND CHARGE FOR REMOVAL IS AN ADDITIONAL COST

IF I, A CLIENT OF JACKIE GOMEZ, NEED TO RESCHEDULE I MUST GIVE AT LEAST 48-HOUR NOTICE PRIOR TO APPOINTMENT. FAILING TO RESCHEDULE BEFORE 48 HOURS WILL RESULT IN FORFEIT OF DEPOSIT. NO-SHOW APPOINTMENTS WILL RESULT IN FORFEIT OF DEPOSIT.
